Terms of Service — Forms Locator

Forms Locator is a Google Forms™ add-on provided by Maxi Research. By using Forms Locator, you agree to these Terms of Service.

1. Using Forms Locator

Forms Locator allows form owners to add location collection to their Google Forms™. When activated, form submitters can use the location link in the confirmation message to submit their current location and an optional note.

To use Forms Locator, the form must be linked to a Google Sheet for its responses. The response Sheet must also be shared with Forms Locator so that location information can be added to the response data.

Forms Locator is provided free of charge.

2. Location matching

Forms Locator uses the form response timestamp to associate a location submission with the corresponding recent response.

The location link is valid for 1 minute after the response is submitted. Because location matching relies on timing, closely timed submissions can affect the association. Form submitters can add an optional note to help identify their submission when needed

3. Location accuracy

Location accuracy depends on the respondent’s device, browser, network, GPS availability, and other factors. Mobile devices can provide more accurate location information than laptops or desktop computers because they can use GPS and other device-based location services.

Forms Locator does not guarantee the accuracy of location information provided.

4. User responsibility

The form owner is responsible for how Forms Locator is used, including informing form submitters about location collection and complying with applicable laws and requirements.

Form submitters choose whether to provide their location and any optional note.

5. Service availability

Forms Locator is provided on an “as is” and “as available” basis. We do not guarantee uninterrupted or error-free operation or that every location submission will be successfully collected or matched.

Maxi Research can modify, suspend, or discontinue Forms Locator or any part of the service.

6. Changes and contact

We can update these Terms of Service from time to time.
